health condition for purposes of underwriting, claims
processing, and such other purposes as you may authorize,

We do not disclose non-public personal
information about our customers or former customers
to non-affiliated third parties except as permitted
by law or as authorized by you in writing.
We may disclose, as allowed by law,

we agree, we will correct our files. If we do not
